Working as a team with Deb, clients explore the root causes of their symptoms through advanced laboratory testing, imaging, and detailed tracking of nutrition, lifestyle, and environment. Together, this information forms a comprehensive roadmap toward optimal health, with Deb walking alongside each client every step of the way.
As a small independent practice, Instinctive Wellness operates as a cooperative between providers and patients. By minimizing administrative overhead and front desk staff, Instinctive Wellness is able to offer highly effective patient visits at a fraction of the cost. This allows us to focus on building the tools and plans that truly work for you.
COMMUNICATION / APPOINTMENTS
To ensure secure and efficient communication, all correspondence and appointment scheduling must be conducted through the Athena Patient Portal, as Deb does not employ a receptionist. In accordance with HIPAA regulations, email communication outside of the portal is not permitted. The self-scheduling feature within the portal offers the simplest and most streamlined way to request an appointment.
While phone calls or emails may feel quicker, using the secure patient portal ensures that your message is automatically documented as part of your medical record. Communications received outside the portal require additional handling and increase the risk of delays, miscommunication, or incomplete documentation. To ensure safe, timely, and well-documented care, all patient communication must take place through the secure patient portal. Communications received by phone or email regarding patient care will not be responded to directly.
For non-urgent matters, you can expect a response via the portal within 48 hours during standard business hours. If your concern requires immediate attention, please indicate the urgency in your message.
In the event of an emergency, please contact your primary care provider’s on-call service or proceed to an emergency facility.
Deb Reid is happy to address quick questions through the Patient Portal at no charge. However, messages that are complex or require clinical decision-making—such as guidance on medications, supplements, treatments, additional lab testing, or new health concerns—will be billed at a rate of $199/hour, prorated based on the time required to address the concern. If an issue is best handled through a full visit, Deb will inform the patient accordingly.
Instinctive Wellness emphasizes that individualized care cannot be delivered through prepackaged or generic responses. Patients deserve thoughtful, tailored guidance. When used appropriately, secure messaging allows for more timely, direct communication and can support better health outcomes between visits.
PRIMARY CARE PROVIDER
It is important to maintain your relationship with your primary care provider, as Instinctive Wellness does not offer all the services they provide. You are welcome to share copies of your visit notes and completed tests from this practice with your primary care physician. These documents are accessible through the patient portal.
Instinctive Wellness does not offer:
- Well woman exams
- Screening tests or coordination of screening tests like colonoscopy, etc.
- Vaccines
- Surgical Clearance
- Medicare Wellness Exams
- Sports/School physicals
- DOT physicals
- Refills for chronic conditions unless the medication was started by Instinctive Wellness
- A 24-hour on-call line
- Hospital admissions
IN PERSON AND TELEMEDICINE APPOINTMENTS
Deb offers both in-person and telemedicine appointments. If you choose to have a telemedicine appointment, please review the following information about the pros and cons and potential risks. Telemedicine involves the use of electronic communications to enable health care providers at different locations to share individual patients’ medical information for the purpose of improving patient care. The information may be used for diagnosis, therapy, follow-up and/or education.
Telemedicine may include any of the following:
- Patient medical records
- Medical images
- Live two-way audio and video
- Output data from medical devices and sound and video files
Electronic systems used will incorporate network and software security protocols to protect the confidentiality of patient identification and imaging data and will include measures to safeguard the data and to ensure its integrity against intentional or unintentional corruption.
Expected Benefits:
- Improved access to medical care by enabling a patient to remain in his/her home
- Decreased exposure to infections in the office
- More efficient medical evaluation and management.
Possible Risks:
- As with any medical procedure, there are potential risks associated with the use of telemedicine.
- These risks include, but may not be limited to:
- In rare cases, information transmitted may not be sufficient (e.g. poor resolution of images) to allow for appropriate medical decision-making by the provider(s)
- Delays in medical evaluation and treatment could occur due to deficiencies or failures of the equipment
- In very rare instances, security protocols could fail, causing a breach of privacy of personal medical information
- In rare cases, a lack of access to complete medical records may result in adverse drug interactions or allergic reactions or other judgment errors
By signing this form, you understand the following:
- You understand that the laws that protect privacy and the confidentiality of medical information also apply to telemedicine, and that no information obtained in the use of telemedicine which identifies you will be disclosed to researchers or other entities without your consent.
- You understand that you have the right to withhold or withdraw your consent for the use of telemedicine in the course of your care at any time, without affecting your right to future care or treatment.
- You understand that you have the right to inspect all information obtained and recorded in the course of a telemedicine interaction, and may receive copies of this information for a reasonable fee.
- You understand that a variety of alternative methods of medical care may be available to you, and that you may choose one or more of these at any time.
- You understand that telemedicine may involve electronic communication of your personal medical information to other medical practitioners who may be located in other areas, including out of state.
- You understand that it is your duty to inform your provider of electronic interactions regarding care that you may have with other healthcare providers.
- You understand that you may expect the anticipated benefits from the use of telemedicine in your care, but that no results can be guaranteed or assured.
Your Signature below indicates that you have read and understand the information provided above regarding telemedicine, have discussed it with your provider and all of your questions have been answered to your satisfaction. Your signature below hereby gives informed consent for the use of telemedicine in your medical care and that you hereby authorize the providers of Instinctive Wellness to use telemedicine during your diagnosis and treatment.
CONSENT FOR USE OF AN AI SCRIBE
To enhance the quality of care and improve documentation accuracy during your medical visits, this practice utilizes an Artificial Intelligence (AI) scribe. Below are key details about how the AI scribe works and your rights:
Purpose of the AI Scribe
The AI scribe assists in documenting medical information discussed during your visit. It helps ensure your medical records are accurate and comprehensive while allowing your provider to focus more on you during the visit.
How the AI Scribe Works
The AI listens to and transcribes portions of the visit in real time, recording relevant medical information discussed between you and your provider. These recordings are securely processed and deleted once the notes are finalized.
Privacy and Security
The AI scribe complies with HIPAA regulations to protect your health information. All data is encrypted during transmission and storage. Only authorized personnel have access to your medical records.
Your Consent
By proceeding with the visit, you acknowledge the use of the AI scribe. If you have concerns or prefer not to use this technology, please inform us. Instinctive Wellness will accommodate your request, and your care will not be impacted.
Use of AI Scribe – Impact on Visit Time
The AI scribe is used to support accurate documentation and allow visits to focus on clinical discussion rather than manual note-taking. Patients may decline the use of the AI scribe at any time. In such cases, alternative documentation methods will be used. Please note that manual documentation may increase visit length and associated time-based fees, consistent with the practice’s standard billing structure. All efforts will be made to ensure care remains patient-centered, transparent, and collaborative regardless of documentation method.
Patient Rights
You may review and request corrections to your medical records at any time. If you have questions or concerns about the AI scribe, please speak with your provider via the portal.

CONSENT FOR USE AND DISCLOSURE OF PROTECTED HEALTH INFORMATION
You hereby give your consent for Instinctive Wellness, LLC to use and disclose protected health information (PHI) about yourself to carry out treatment, payment and healthcare operations (TPO). The Notice of Privacy Practices provided by Instinctive Wellness describes such uses and disclosures more completely.
You have the right to review the Notice of Privacy Practices prior to signing this consent. Instinctive Wellness reserves the right to revise its Notice of Privacy Practices at any time. A revised Notice of Privacy Practices may be obtained by forwarding a written request to:

With this consent, Instinctive Wellness may call your home or other alternative location and leave a message on voice mail or in person regarding any items that assist the practice in carrying out TPO, such as appointment reminders, insurance items and any calls pertaining to your clinical care, including laboratory test results, among others.
With this consent, Instinctive Wellness may mail to your home or other alternative location any items that assist the practice in carrying out TPO, such as appointment reminder cards and patient statements as long as they are marked “Personal and Confidential.”
With this consent, Instinctive Wellness may send text messages to your personal cell phone or other alternative location any items that assist the practice in carrying out TPO, such as appointment reminder cards and patient portal notifications.
You have the right to request that Instinctive Wellness restrict how it uses or discloses your PHI to carry out TPO. The practice is not required to agree to your requested restrictions, but if it does, it is bound by this agreement. You understand that emails are not secure as they are not encrypted.
By signing this form, you are consenting to allow Instinctive Wellness to use and disclose your PHI to carry out TPO.
You may revoke your consent in writing except to the extent that the practice has already made disclosures in reliance upon your prior consent. If you do not sign this consent, or later revoke it, Instinctive Wellness will not decline to provide treatment to you.
DOCUMENT HANDLING
Medical Record Requests
Document handling done by this office is very time-consuming and can often cut into clinical time with patients, so please strive to maintain your own records whenever possible. Please check the patient portal first as all of the necessary records are available already. If indeed records are requested to be sent to another provider, document handling fees will be processed at the rate determined by the state of Maryland at a rate of $0.76 per page for paper records (this includes fees, postage and handling). When medical records are requested digitally on a patient’s behalf, a setup fee of $22.88 plus $ 0.57 per page applies per Maryland statutes, reflecting the time required to request, receive, review, and organize records from external systems. Additional costs will apply to cover the costs of external storage media if this is the preferred format. Medical record requests will be processed as soon as possible but note that this may take as much as 21 days to complete. Fees are assessed in accordance with Maryland state law and are subject to change if statutory rates are updated.
Providing Medical Records
Managing your own records from other providers can be time-consuming, confusing, and frustrating. There are medical record apps, such as Apple Health or other Android options, that can easily help you share your records with this office or others. Also, if you are overwhelmed with managing your records before your appointment, you can schedule a quick appointment with Deb Reid so you can work together to grab all the needed records from all of the portals you have. This process requires your login information and that you are able to log in to each portal. This is typically only needed for initial appointments for those who have extensive medical records that should be reviewed prior to the appointment. If you do not have extensive records, you can upload those records in an organized fashion to the patient portal via a message. Either way, it is best to send in records and complete all forms ahead of time to reduce the time and cost of the visit.
Special forms
Completing paperwork for schools, camps, the Family Medical Leave Act (FMLA) claims, long-term care, life insurance, the Department of Veterans’ Affairs, disability claims, HSA/FSA justifications or other purposes requires extra work on the part of Instinctive Wellness and goes beyond routine medical care. The charge is $49.75 per form for up to 3 pages. Beyond that, it is an additional $5 per page.
PAYMENT OPTIONS:
All clients are expected to pay in full at the time of service. Currently, only electronic payment formats are accepted such as credit / debit cards as well as HSA / FSA funds. Payment by check or cash are not accepted. Additionally, Instinctive Wellness works with medical credit cards such as Advanced Care and Care Credit, both of which offer low or no interest payments for medical bills, depending on your credit score.
Card on File Agreement
At Instinctive Wellness, Deb Reid strives to make the billing process as simple and efficient as possible. To ensure seamless service, patients are required to maintain a credit card on file for billing purposes. The following outlines how this process works and what patients can expect:
Authorization Details
By providing your payment information, you authorize Instinctive Wellness to charge your payment method for any outstanding balances related to services and fees incurred. You will receive notification with a 5-day window to adjust the payment method for any charges that happen outside of a visit.
You will receive a card on file agreement via text message for you to review and approve prior to starting this agreement.
- Your card will only be charged for your initial visit deposit, at the end of your visits or for care between visit fees as agreed upon as well as no shows and late cancellation fees.
- A receipt will be provided for each transaction, detailing the amount charged.
- If the total charge exceeds the agreed-upon maximum amount, you will be notified and billed any remaining balance directly.
- Your payment information will be securely stored using industry-standard encryption through this trusted payment processing partner, Elavon.
- This ensures that your sensitive financial information is protected at all times.
- You may cancel this authorization at any time by contacting the office via the portal. Upon cancellation, you will be responsible for paying outstanding balances directly.
- Any changes you make to your payment method, or this authorization will supersede prior agreements.
Acknowledgment
By providing your payment information, you confirm that you are an authorized user of the payment method provided. All details in this authorization are true and accurate. If you have questions or need to update your card on file, please contact us via the portal.
FUNCTIONAL MEDICINE CONSULTS AND FEES
Initial Visit
- 2 hours: $398
- Beyond 2 hours, time is prorated up in 15-minute increments at $49.75 per block
- Your $199 (50%) patient deposit will be applied to the total visit cost and is non-refundable.

Billed visit time reflects both direct and indirect care, like documenting the visits and orders, required for safe and thorough treatment.
The Core Healing Program Option
The Core Healing Program is 6-month program designed for clients who want high-touch, ongoing support with savings built in! Over the six-month period, new clients will save $328, and established patients will save $129, PLUS any additional savings from the 15% discount on extra services. And you can renew as often as it works for you!

Refill Requests, Lab/Test Reorders, and Prior Authorizations
Refills requested outside of a visit: $49.50.
Re-Generating / Changing Clinical Orders (Labs, Imaging, Referrals)
Updating expired orders (typically valid for 6 months): $49.75. Switching lab company or provider, changing or canceling orders or referrals after they have been placed: $99.50–$199 based on time and complexity.
To avoid additional fees or delays, please notify us in advance if you prefer a specific lab or referral destination.
Using Labs Other Than Quest or LabCorp
You may use any lab; however:
- Hospital labs often lack full access to specialized tests and may delay results
- Lab staff frequently miscode / omit tests, even when drawing blood for outside labs
- Manual data entry is required for labs processed outside Quest or LabCorp
- This results in extra clinical time and increased risk of error
Fee for manual lab entry:
$99.50 per 10 lab results, prorated accordingly. (Most new patient panels include 35–55 labs)
It is strongly recommended that you use LabCorp or Quest whenever possible.
Messaging Fees
Secure messaging is a valuable tool for between-visit care, but messages requiring clinical input are time-based services.
- Medical messages requiring review or decision-making: $199/hour
- Prorated in 15-minute blocks
- Most messages: $49.75
- More complex messages requiring >15 minutes may be billed at a higher amount
- No charge for administrative-only messages (e.g., “Where are my orders?”)
- Will be notified via portal message before any billable time is incurred whenever feasible
Research & In-Depth Record Review
Functional and investigative care may require extensive outside medical record review and in-depth research into complex, layered conditions. If additional time beyond 30 minutes is anticipated, you will receive advance notification and a cost estimate. This ensures transparency and avoids across-the-board rate increases.
Genetic Report Review And Interpretation
For clients who have completed genetic testing through services like 23andMe or AncestryDNA, Deb offers an in-depth review that goes far beyond the limited summaries provided by those platforms. Due to FDA regulations, companies like 23andMe are restricted in the health information they can disclose. By uploading your raw data to Promethease—a continuously updated database of gene-related research—Deb can generate a comprehensive, clinically relevant report tailored to your health concerns. This analysis incorporates the latest findings on how specific genes may impact your health, risk factors, and functional pathways. Once generated, your personalized genetic report will be uploaded to your chart and reviewed at your next visit.
Cost for analysis and report preparation: $148.25
Record Gathering For Complex Cases
For clients with extensive medical histories across multiple hospital systems, Deb offers support in gathering and consolidating records. During a brief video call, the client provides access (such as share codes, not personal portal log-ins), allowing Deb to securely log into each applicable health system and download the necessary records directly—saving the client from the time-consuming task of printing or uploading documents manually.
This service is particularly helpful before initial visits or complex follow-ups where comprehensive review is essential.

Prior Authorizations
- Prior authorizations are a common but often time-intensive administrative task. Some can be completed in as little as 15–20 minutes, while others may require extensive follow-up, documentation, and appeals.
- There is no fee for prior authorizations that can be completed within 15–20 minutes.
- If additional time is required, prior authorizations will be billed at $199/hour, prorated in 15-minute increments, based on the actual time spent.
- You will always be notified in advance with a time estimate and cost for your approval before the process begins.
- If a prior authorization must be appealed or escalated for further review, additional charges may apply following the same time-based rate and advance notification process.
- The complex prior authorizations are rare and typically only for specialty medications however, insurance dictates that process entirely.
SELF-PAY & PATIENT-SUBMITTED INSURANCE REIMBURSEMENT (SUPERBILLS)
Instinctive Wellness will provide you with a receipt for all fees paid as well as an itemized superbill for the visits to send to your insurance company if you have out-of-network benefits. The superbill will have all the information necessary for the insurance company to determine potential reimbursement. Instinctive Wellness cannot guarantee when or the amount you will be reimbursed as this is dependent on several factors including whether your plan provides any coverage for out-of-network providers as well as deductibles.
Depending on your insurance policy, you may also need to obtain a referral from your primary care doctor prior to being seen. Prior to your visit, it may be best to call your insurance company as they are often able to provide you with a more accurate estimate of your out-of-pocket costs and the need for a referral.
Instinctive Wellness, LLC will make every effort to maximize what your insurance will cover; but cannot guarantee insurance coverage of services. This is an important consideration as Functional Medicine is a newer field that utilizes the most current medical science to provide cutting-edge care. However, Functional Medicine and its use of innovative diagnostic testing has not yet gained widespread acceptance in the insurance community. Therefore, there may be instances where the insurance company is not willing to pay for either the testing or the cost of the visits.
Should a client run into financial difficulty or wants to transition to another provider, Instinctive Wellness, LLC will work with the client to help transition their care as well as any medication management to another provider as necessary. In cases such as this, Instinctive Wellness, LLC will continue to provide medication refills for the client for up to 3 months to allow the client to establish care with another provider, if this is in keeping with best practice for the medication in question.
SUPPLEMENT & FULLSCRIPT DISPENSARY POLICY
Instinctive Wellness may recommend specific nutritional supplements as part of individualized care. Patients are not required to purchase supplements through any specific vendor and are encouraged to obtain recommended products in the most affordable way available, provided the formulation and manufacturer match what was recommended. Nutrition and access to high-quality food are foundational to health, and supplement costs should never interfere with a patient’s ability to eat well.
For clarity and safety, Instinctive Wellness uses a curated online dispensary to identify exact supplement formulations and manufacturers. The practice may receive a modest financial benefit from purchases made through this platform, which helps offset the clinical time required for supplement evaluation, safety screening, and ongoing management. Financial considerations do not influence which supplements are recommended, and patients retain full autonomy over where they purchase products.
Certain supplements, such as fish oil and probiotics, may be recommended for purchase directly through the dispensary or manufacturer due to storage and handling considerations that affect product quality.
INITIAL APPOINTMENTS FORMS POLICY
Initial functional medicine appointments require completion of all Google Symptom Forms and the Self Check-In.
Forms must be received at least 4 days (96 hours) prior to the scheduled initial appointment. If required forms are not received by this deadline, the appointment will move toward cancellation under the practice’s cancellation policy.
In this situation, you may choose one of the following:
Cancel & Reschedule
- If rescheduled within 7 days, your $199 deposit will be applied to the new appointment.
- If not rescheduled within 7 days, the deposit will be applied toward the applicable late-cancellation fee.

Proceed Without Forms
- You may choose to proceed without completed forms.
- Initial visits without forms require substantially more time and often last 6 hours or more.
- Time will be billed at the standard rate of $199/hour and due at the time of the visit
Follow-up appointments do not require Google Symptom Forms.
CANCELLATION & MISSED VISIT POLICY (NO SHOWS)
Less than 72 hours’ notice or no shows result in the full visit cost as a cancellation fee:
- $199 for follow-up visits
- $398 for initial visits
Within 7 days results in a 50% cancellation fee:
- $99.50 for follow-ups
- $199 for initial visits
Emergency exceptions: ER visits, hospitalizations, etc.
MEDICARE / TRICARE PRIVATE CONTRACT (Applies Only to Those With Medicare or Tricare in compliance with 42 U.S.C. §1395a; 42 C.F.R. § 405, SUBPART D)
This contract is entered into by and between Deborah Reid / Instinctive Wellness whose principal medical office is located at 312 Park Row, Chestertown, MD 21620 and the Medicare or Tricare beneficiary who has signed below and shall become effective on this 1st day of December 2021 and shall expire on the 1st day of December, 2025.
Provider Obligations
The Provider acknowledges that she is excluded aka “opted out” from Medicare under sections 1128, 1156, 1892 or any other section of the Social Security Act. The provider acknowledges that she is a non-network and non-participating Tricare certified provider. The Provider acknowledges that this contract shall not be entered into with the beneficiary, or the beneficiary’s legal representative, during a time when the beneficiary requires emergency care services or urgent care services, except that the Provider may furnish emergency or urgent care services to a Medicare or Tricare beneficiary in accordance with 42 C.F.R. § 405.440. The Provider acknowledges that she must retain this contract for the duration of the opt-out period, and that it shall be made available to the Centers for Medicare and Medicaid Services (CMS) or Tricare intermediaries upon request. The Provider shall provide a copy of this contract to the beneficiary, or to his or her legal representative before items or services have been furnished to the beneficiary under the terms of this contract. The Provider acknowledges that she must enter into a contract for each opt-out period.
Beneficiary Obligations
The beneficiary, or his or her legal representative, accepts full responsibility for payment of the Provider’s charge for all services furnished by the Provider. The beneficiary, or his or her legal representative, understands that no payment will be provided by Medicare for items or services furnished by the Provider that would have otherwise been covered by Medicare if there was no private contract and a proper Medicare claim had been submitted. The beneficiary, or his or her legal representative, understands that Medicare or Tricare limits do not apply to what the Provider may charge for items or services furnished by the Provider. The beneficiary, or his or her legal representative, AGREES NOT TO SUBMIT A CLAIM, nor ask the Provider to submit a claim, to Medicare or Tricare, even if such items or services are otherwise covered by Medicare or Tricare. The beneficiary acknowledges that this written private contract contains sufficiently large print to ensure that the beneficiary is able to read this contract.
The beneficiary, or his or her legal representative, has entered into this contract with the knowledge that he or she has the right to obtain Medicare-covered items or Tricare covered items and services from other Providers and practitioners who have not opted-out of Medicare or Tricare and for whom payment would be made by Medicare or Tricare for their covered services, and that the beneficiary has not been compelled to enter into private contracts that apply to other Medicare/ Tricare -covered services furnished by other Providers or practitioners who have not opted-out. The beneficiary, or his or her legal representative, understands that Medigap plans do not, and other supplemental plans may elect not to, make payments for items and services not paid for by Medicare.
For Tricare Prime beneficiaries who have the option for “Point of Service” benefits, once you have paid the full amount for each visit, you may submit a superbill to your local Tricare intermediary for possible reimbursement (minus your cost share) based upon your benefits plan. The provider has the legal right to charge up to 15% more than the TRICARE allowable charge. Any charges above the allowable charge are your responsibility and will not be reimbursed.
The beneficiary, or his or her legal representative, understands that this agreement shall not be entered into with the Provider during a time when the beneficiary requires emergency care services or urgent care services, except that the Provider may furnish emergency or urgent care services to a Medicare/Tricare beneficiary in accordance with 42 C.F.R. § 405.440.
The beneficiary, or his or her legal representative, acknowledges that a copy of this contract has been provided to the beneficiary, or to his or her legal representative, before items or services have been furnished to the beneficiary under the terms of this contract.
For Medicare beneficiaries, you understand that during the opt-out period, a Medicare Advantage plan may not by law make any payments to the Provider for any Medicare items and services furnished to the beneficiary under this contract.

FUNCTIONAL MEDICINE SERVICES INFORMED CONSENT AND LIABILITY WAIVER
By signing this consent, you agree to receive services for yourself or your dependent from Deborah Reid under the auspices of Instinctive Wellness, LLC. These services include:
- Past Medical Chart Review and Summary
- Extensive Medical History and Physical Examination
- Laboratory evaluation, including in office venipuncture
- Medical Nutrition and Supplements as needed
- Lifestyle and Activity Recommendations
- Hormone Balancing
- Detoxification through nutrition and supplements as needed

Should any changes to your regimen be suggested by Deborah Reid and/or Instinctive Wellness, LLC, you should consult with your primary physician prior to making any changes. Your signature below is a statement of your understanding and acknowledge of the need to maintain and follow your primary physician’s advice.
Should any urgent or emergent medical condition occur, please contact your primary physician immediately or access emergency care as necessary. This office does not provide after-hours care or on-call services.
By signing this consent, you also indicate understanding and acceptance of the inherent risks regarding medical laboratory testing, venipuncture, lifestyle changes including increased physical activity as well as medical nutrition with supplementation and prescription medication as needed. These risks include the remote possibility of death or significant morbidity as is the case with any medical intervention. Also, as with any venipuncture, there exists the risk of bruising, bleeding, hematoma as well as temporary or permanent nerve or tendon damage.
Additionally, your signature below indicates your understanding and acceptance that Instinctive Wellness, LLC and/or Deborah Reid do not guarantee any specific outcomes.
TELEMEDICINE AND STATE LICENSURE
Maryland
Deborah Reid, FNP-BC, is licensed to practice as a Family Nurse Practitioner in the state of Maryland. Telehealth services may be used to initiate care for patients located in Maryland. An initial in-person visit is recommended whenever feasible, as it helps establish a solid clinical baseline . If an in-person visit is not possible due to distance or other constraints, care may proceed via telemedicine, and coordination with local providers may be recommended when clinically appropriate.
In-person evaluations may also be recommended at any point during care if clinically indicated.
Whether visits are conducted in person or via telemedicine, patients must be physically located within the state of Maryland at the time of the visit unless otherwise permitted under applicable interstate telehealth provisions.
Delaware (Interstate Telehealth Services)
Under Delaware law, out-of-state healthcare providers may offer telehealth services to Delaware residents through an Interstate Telehealth Registration, after a provider–patient relationship has been established through an initial in-person visit. Accordingly, Instinctive Wellness provides care to Delaware residents only after an initial in-person visit has been completed at the practice’s Maryland office, which establishes both the provider–patient relationship and a clinical baseline, as required by Delaware regulations.
Following that initial in-person visit, telemedicine services may continue for Delaware residents in accordance with Delaware’s interstate telehealth requirements. Depending on clinical circumstances, additional in-person visits may be recommended. When travel to the Maryland office is not feasible or clinically necessary, coordination with or referral to a local provider may be recommended to ensure appropriate and timely care.
Patients must be physically located within the state of Delaware at the time of any telemedicine visit for this authorization to apply.

Nature of Reiki Services
Reiki is a Japanese energy healing practice that supports physical, emotional, mental, and spiritual well-being by facilitating the body’s natural self-healing processes. During a Reiki session, the client remains fully clothed while the practitioner places hands gently on or above the body. There is no massage, manipulation, or intimate touch.
Reiki sessions may include intuitive or energetic insights. These insights are offered as reflective and supportive information only and are not diagnostic, predictive, or prescriptive in nature. Clients retain full autonomy in how they interpret and integrate their experience.
No specific outcomes are guaranteed. Reiki does not impose healing or change; it supports and augments the client’s own innate healing capacity.
Scope of Practice & Medical Disclaimer
Deborah Reid is a Board-Certified Family Nurse Practitioner. Reiki services are provided as a complementary wellness modality within her professional practice and ethical standards. Reiki does not involve medical diagnosis, medical treatment, or prescribing unless explicitly scheduled and documented as a separate medical visit.
Reiki is a complementary wellness modality and is not a substitute for medical care, mental health care, or emergency services.
Reiki:
- Does not diagnose or treat disease
- Does not prescribe medications or supplements
- Does not replace care from a primary care provider or specialist
Clients are encouraged to maintain an ongoing relationship with their primary care provider and to seek appropriate medical or mental health care when needed.
COMMUNICATION / APPOINTMENTS
To ensure secure and efficient communication, all correspondence and appointment scheduling must be conducted through the Athena Patient Portal, as Deb does not employ a receptionist. In accordance with HIPAA regulations, email communication outside of the portal is not permitted. The self-scheduling feature within the portal offers the simplest and most streamlined way to request an appointment.
While phone calls or emails may feel quicker, using the secure patient portal ensures that your message is automatically documented as part of your record. Communications received outside the portal require additional handling and increase the risk of delays, miscommunication, or incomplete documentation. To ensure safe, timely, and well-documented care, all client communication must take place through the secure patient portal. Communications received by phone or email regarding care will not be responded to directly.
For non-urgent matters, you can expect a response via the portal within 48 hours during standard business hours. If your concern requires immediate attention, please indicate the urgency in your message.
In the event of an emergency, please contact your primary care provider’s on-call service or proceed to an emergency facility.
STRUCTURE OF REIKI SESSIONS AND FEES
Reiki Session Format
- Reiki sessions are scheduled as 90-minute appointment slots, which include:
- Time beforehand to discuss intentions, expectations, and grounding
- The Reiki session itself
- Time afterward to discuss insights or experiences that arose during the session
- Time to develop a plan for daily at home interventions to maintain energetic health
Fee
90-minute Reiki session: $110
All services are private pay and due at the time of service.
PAYMENT OPTIONS & CARD ON FILE AGREEMENT
All clients are expected to pay in full at the time of service. Currently, only electronic payment formats are accepted such as credit cards as well as HSA / FSA funds are accepted. Payment by check or cash are not accepted.
Card on File Agreement
At Instinctive Wellness, Deb Reid strives to make the billing process as simple and efficient as possible. To ensure seamless service, clients are required to maintain a credit card on file for billing purposes. The following outlines how this process works and what clients can expect:
Authorization Details
By providing your payment information, you authorize Instinctive Wellness to charge your payment method for any outstanding balances related to services and fees incurred. You will receive notification with a 5-day window to adjust the payment method for any charges that happen outside of a visit.
You will receive a card on file agreement via text message for you to review and approve prior to starting this agreement.
Your card will only be charged for your initial visit deposit (where required) and at the end of your visits, as well as for no-shows and late cancellation fees.
A receipt will be provided for each transaction, detailing the amount charged.
If the total charge exceeds the agreed-upon maximum amount, you will be notified and billed any remaining balance directly.
Your payment information will be securely stored using industry-standard encryption through this trusted payment processing partner, Elavon. This ensures that your sensitive financial information is protected at all times.
You may cancel this authorization at any time by contacting the office via the patient portal. Upon cancellation, you will be responsible for paying outstanding balances directly.
Any changes you make to your payment method or this authorization will supersede prior agreements.
Acknowledgment
By providing your payment information, you confirm that you are an authorized user of the payment method provided. All details in this authorization are true and accurate.

Insurance & Reimbursement
Reiki is not covered by insurance. Superbills are not provided for Reiki services.
Some HSA or FSA plans may allow reimbursement for complementary wellness services; eligibility is determined solely by the plan administrator. Clients are responsible for verifying eligibility with their plan. If a letter of medical necessity is required, one can be provided for a one-time fee of $49.75.
CONFIDENTIALITY & RECORDS
All information shared during Reiki sessions is treated with the same level of confidentiality and data security as medical services provided by Instinctive Wellness.
Reiki session notes, when applicable, are stored securely and may be accessed through the patient portal for review or download.
BOUNDARIES OF REIKI SERVICES
Reiki sessions are time-limited, contained services and do not include:
- Crisis intervention or emergency support
- Ongoing emotional processing outside the session
- Psychiatric care or therapy
- On-call availability or between-session support
If concerns arise that exceed the appropriate scope of Reiki, referral to medical, psychiatric, or therapeutic care may be recommended.
Appropriateness of Reiki Services
Reiki may not be appropriate for individuals experiencing active dissociation, unmanaged severe psychiatric symptoms, or conditions that impair physical safety, reality testing, or self-regulation during altered states. Please discuss this with Deb ahead of time if there are any possible concerns.
Instinctive Wellness reserves the right to pause, modify, or discontinue Reiki services if a session becomes unsafe or exceeds the appropriate scope of Reiki practice. When clinically or ethically indicated, referral to appropriate medical or mental health support may be recommended.
CANCELLATIONS & MISSED VISIT POLICY (No Shows)
Less than 72 hours’ notice or no shows result in the full visit cost.
Within 7 days results in a 50% cancellation fee.
Emergency exceptions apply (ER visits, hospitalizations, etc.).
CONSENT, AUTONOMY & CLIENT RESPONSIBILITY
By signing this agreement, you acknowledge that:
- Reiki is a voluntary, complementary wellness service.
- You remain fully responsible for your physical, emotional, and spiritual well-being.
- You may pause or discontinue a session at any time.
- Intuitive insights are not directives and should not replace professional medical or mental health advice.
Liability Waiver
By signing below, you voluntarily consent to receive Reiki services from Deborah Reid under the auspices of Instinctive Wellness, LLC and agree to indemnify, waive, and release Instinctive Wellness, LLC and Deborah Reid from any and all liability arising from participation in Reiki services, except in cases of gross negligence or willful misconduct.
Any disputes shall be resolved through binding arbitration in accordance with applicable law.
